Acne Scars in Your 20s & 30s: What Actually Works

Acne scars are one of the most emotionally difficult skin concerns — and one of the most commonly under-treated. Many patients spend years covering them with makeup or accepting them as permanent, when in reality, today's dermatological treatments can dramatically reduce even the deepest scars.

Understanding Your Scar Type

Treatment depends entirely on scar morphology. The main types:

  • Ice-pick scars: Deep, narrow, V-shaped scars that extend into the dermis. Hardest to treat — require TCA CROSS (chemical reconstruction of skin scars).
  • Boxcar scars: Broad, shallow-to-medium, U-shaped depressions with well-defined edges. Respond excellently to fractional laser and subcision.
  • Rolling scars: Broad depressions with sloped edges that give the skin a wavy appearance. Caused by fibrous bands tethering the dermis — subcision is the gold-standard treatment.
  • Hypertrophic / Keloid scars: Raised scars from excess collagen. Treated with intralesional steroid injections and laser.
  • PIH (Post-Inflammatory Hyperpigmentation): Flat dark marks — technically not scars, but often mistaken for them. Respond excellently to Q-Switch laser, peels, and brightening protocols.

The Treatments That Actually Work

Fractional CO2 Laser: The most powerful resurfacing tool available. Ablates damaged skin in a controlled pattern, stimulating significant collagen remodelling. 3–5 sessions with 4–6 weeks between each typically achieve 50–70% improvement in boxcar and rolling scars. Requires 5–7 days downtime per session.

Subcision: A needle is inserted under rolling scars to cut the fibrous bands tethering the scar to deeper tissue. Often the most dramatic single-treatment improvement for rolling scars. Usually combined with filler or PRP to prevent re-attachment.

TCA CROSS: High-concentration TCA is applied precisely to the base of ice-pick scars with a toothpick, causing controlled destruction and regrowth of healthier tissue within the scar column. 3–5 monthly sessions typical.

PRP Micro-needling: Micro-needles create controlled wound channels that trigger collagen synthesis; simultaneously PRP is infused to accelerate healing. Excellent for diffuse shallow scarring and PIH. Minimal downtime — redness for 24 hours.
